This is a new feature we're currently working on and testing. It's going through an accelerated roll out because NaNoWriMo starts in a few days. Please let me know if you see any bumps or bruises along the way.

Users don't have to mark content as NSFW or 18+ and I guarantee that many users don't (it's among least used features we have). If I implement some sort of support for adult content (cutting the entries probably), 99% of the time you're still going to see the adult content.
